Our Mission

Truecare Home Care is a full service Home care Agency, licensed by Georgia Department of Community Health (DCH). We offer personal care services, nursing services, companion/sitter and other health services at home to help the elderly and disabled maintain their independence with dignity and respect.  We provide free evaluation and tailor the care to the need of the individual and his or her family.

Our care providers include Caregivers, Certified Nursing Assistants (CNAs), Personal Care Assistants (PCAs), Registered Nurses (RNs) and Physicians (MDs). Our services are from two (2) hours to twenty four (24) hours per day, seven (7) days a week.

All our care professionals are thoroughly screened and tested by us and approved screening center.  Our service areas include the greater Atlanta areas including DeKalb, Fulton, Gwinnett, Cobb, Clayton, Rockdale, Henry, Douglass and Newton counties.

When you call Truecare home care, a specialist in homecare will be glad to answer your questions and address any concerns that you may have.

A Truecare specialist will also meet with you in person to discuss your needs.  Upon receipt of your approval, a registered nurse will begin services and conduct an initial consultation. All findings and requests are then incorporated into Truecare home care approved plan of care.

You can depend on Truecare home care to assign nursing professionals who are qualified and compassionate and focused on your needs.  We will align your needs to our nurses’ qualifications to ensure compatibility. Our entire nurses undergo thorough interviewing, testing, and screening process prior to employment with Truecare home care services.  Furthermore, nurses are regularly supervised and evaluated by Truecare management team and RN to ensure service standards are met and in compliance with DCH.

We know your needs are unique, so we listen to your concerns and needs with care and empathy and give you our undivided attention.

We begin by reviewing your needs through discussions with involved family member; wen necessary we consult with the Physician (s) of the care recipient, social workers, hospitals, or nursing home staff.

Next, working in conjunction with the family, we develop an individual program to manage your daily needs.  We match your needs to the best possible caregiver, bring them to your home and ask you to participate in the selection process.

This allows the caregiver to become familiar with the client needs and also help build a good relationship between the client and the caregiver.

COMMUNICATIONS

After the caregiver has been placed in your home, we continue to follow up with the client through personalized contacts, telephone check-in, and home visit.

Most importantly, we continuously communicate with the caregiver who is providing the care to you.  Our Client family and our assigned caregiver will continuously discuss the care of plan, review progress of client and determine if changes to the plan is required and recommendation is made.
